Kindergarten children must be five (5) years of age on or before September 1st. First grade children must be six (6) on or before September 1st. Legal proof of age shall be one of the following: birth certificate, baptismal certificate, passport, insurance policy in force for two (2) years, family bible with sworn affidavit.

A11 health exams must be dated within the past year. They can be accepted from an out of state physician. HRS 680 certificates must be issued by a Florida physician. All kindergarten-fifth grade registrants must have at least (two) 2 MMR shots indicated on their HRS 680. All grades must have completed the series of three (3) Hepatitis B vaccines. All Kdgn.,lst, 2nd 3rd & 4th grade students need to show proof of chicken pox disease or have proof of the shot. (Varicella) This is mandatory.

Proof of residency shall include the following: One major utility bill and a voter's registration card, driver's license, lease agreement or warranty deed. Please remember to bring two (2) proofs of residency.

If you are currently living with someone, a notarized statement signed by that person stating that you and your child are living with them and two proofs of their residency are required. Within a reasonable amount of time, you shall provide the office with proof that you are receiving mail at their address. Acceptable proofs shall be a current driver's license, bank statement, and/or current charge card statements. Back to Top
